- Agencies should complete one Microsoft form for each Consumer, using the link
- The form can be completed any day of the week
- MassHealth shares all submissions with Tempus every Wednesday by 2:00 pm
- The Use Agreements are reflected on the next PCM Census Report
- If a Consumer is exempt from EVV, they will not receive PRVs and, therefore, do not need to sign a Use Agreement
